Please complete the following form to ensure your participation as a voter who wants to have their voice be heard by signing petitions and resolutions, completing surveys, being informed and if you can, be a volunteer.

Your demographic information is required to ensure we know it is you who participated in a survey, signed a petition or resolution, wants to be on an email or mail list, or wants to be a volunteer. No personal information will be used or disclosed to any person or organization unless you indicate below. For responses to surveys, your responses will be consolidated and reported in summary to elected officials and candidates. Any comments you may provide in a survey will not be identifiable to you as the source. We respect your privacy and endeavor to protect it.
